Employee Commendations and Complaint Forms

Our information forms were created to afford citizens the opportunity to hand write their concerns with or praise of an officer or employee so the Treasure Island Police may continue to uphold our excellence in professional standards and values.

Citizens should turn in a Compliment or Complaint form by one of the following methods to ensure timely processing:
1. To any on-duty police employee,
2. To the Records main office Monday through Friday between the hours of 8:00am and 4:00pm, located at 180 108th Avenue, Treasure Island, FL 33706, or
3. Via email to police@mytreasureisland.org.

Follow-up and proper procedures with any concerned citizen will always take place, and interviews will occur after these are received.
